An AI-related event that causes or could cause significant harm, regulatory breach, security compromise, fundamental-rights impact, or major service disruption. Serious incidents require defined escalation, evidence preservation, corrective action, and where applicable regulatory notification with severity criteria, notification timelines, legal review, and clear ownership.
EU AI Act Article 73 requires providers of high-risk AI to report serious incidents to market surveillance authorities, generally within 15 days.
The OECD AI Incidents Monitor and AIAAIC Repository track serious AI incidents globally, including the Air Canada chatbot ruling and Tessa eating disorder chatbot suspension.
Under EU AI Act Article 55, GPAI providers with systemic risk must report serious incidents and possible corrective measures to the AI Office without undue delay.
