Action taken to eliminate the cause of a nonconformity and prevent recurrence. For AI governance, this may involve control redesign, retraining, approvals, tooling changes, or accountability updates. Common root causes include unclear ownership, weak intake, poor tooling, lack of training, or incentives that bypass governance.
